Pages of Testimony
“Pages of Testimony” are forms for recording the names of those who perished, their biographical details and their photographs (if they exist) in order to restore to each and every one of the victims their own personal identity, which the Nazis attempted to erase. This individual memorial, which is safeguarded in the Hall of Names at Yad Vashem in Jerusalem, as well as in the Central Database of Shoah Victims’ Names (on the Yad Vashem website), serves as a memorial for generations.

Add Documents & Photos
Q. I found a Page of Testimony for a victim I know, but I have additional information about this victim that is not recorded on that Page of Testimony. Do I need to fill out an additional Page of Testimony?
A. In the case of minor corrections or additions, Yad Vashem Hall of Names staff can make changes by adding the new information but not deleting existing data. Information and copies of the documents it is based on may be submitted online by clicking the link “Attach Image or Documentation:" located in the "Feedback Forms" section of the screen displaying the basic biographic information of the victim.
PLEASE NOTE: the original Page of Testimony will not be altered.
If the information you have about the victim is significantly different from, or more detailed than that presented in the Central Database for Shoah Victims’ Names, then we suggest that you submit a new Page of Testimony. We ask that you attach copies of photographs or documents to the corresponding Pages of Testimony.
Using the "Feedback Forms" online will ensure that the additions will correspond to the exact record you wish to amend. Short documents or a biographical note relating to the individual can also be added to the Pages of Testimony in the same way. More extensive documentation should be sent to the Yad Vashem archive.
Q. I would like to submit a photo for a victim recorded on an existing Page of Testimony. How do I do this?
A. Photographs for existing Pages of Testimony may be submitted online by clicking the link “Attach Image or Documentation" located in the "Feedback Forms" section of the screen displaying the basic biographic information of the victim.
PLEASE NOTE: there is a 2 MB file size limit; JPEG format is preferable
Alternatively, photographs may be sent via email to firstname.lastname@example.org or via regular mail to Yad Vashem, Hall of Names, P.O.B. 3477, Jerusalem 91034, Israel. The name of the victim should be written on the back of the photo (or saved as a file with the victim's name) or on an accompanying piece of paper. It is also helpful if you send a printed copy of the existing Page of Testimony, or more information about the victim or name of the submitter to allow us to match the photo to the correct Page of Testimony.
If you would like to submit a group photograph please specify the individual names of the people in the photograph (if known) together with their exact location in the photograph. It is also helpful to note whether they survived or not. Alternatively, you may attach a photocopy of the photo itself and draw lines out to the names of the people on the margins.
If you know the approximate date when and/or the location where the photograph was taken please include mention with the photo.
Corrections to Existing Pages of Testimony
Corrections to existing Pages of Testimony may be submitted online by clicking the “Comments” link located in the "Feedback Forms" section of the screen displaying the basic biographic information of the victim on the Names Database.
Alternatively, corrections may be sent via email to: email@example.com or via regular mail to Yad Vashem, Hall of Names, P.O.B. 3477, Jerusalem 91034, Israel. The full name of the victim should be written on your application. It is also helpful if you send a printed copy of the existing Page of Testimony, or more information about the victim or the name of the submitter to allow us to locate the correct Page of Testimony. Please add a copy of the document the correction is based on.
If the information on the Page of Testimony is correct, but our deciphering of it is mistaken, you may send us the correct information by clicking the “Correct Our Deciphering " link located at the bottom of the screen displaying the full record details of the victim.
